Step 1: Arrival

• Our clinic is located in the Carlyle Building, near the Stone Arch Bridge and Mississippi River.
• The official address is 150 3rd Ave South, Minneapolis, MN 55401.

Step 2: Use Nearby Public Parking

The Carlyle parking garage is reserved for residents/providers only

• Metered street parking: Available on 3rd Ave S, 2nd St S, and surrounding blocks. Parking ranges from $0.25-$4/hour depending on the events of the day
• Public ramps/garages nearby:
  • Mill District City Ramp (2nd St S & 2nd Ave S) – 2-3 minute walk.
  • St. Anthony Main parking lots across the river – short walk via Stone Arch Bridge.
  • Surface lots along Washington Ave S.

Step 3: Walk to the Carlyle

• From your parking spot, walk to the Carlyle Building main entrance at 150 3rd Ave S.

Step 4: Enter the Clinic

• At the front door keypad, enter the unique code that was texted and emailed to you to confirm your appointment. If you do not have it call the clinic at 651-472-5915.
• Please sign in at the white kiosk so your provider knows you have arrived
• If you need to complete intake paperwork, these can be found next to the white kiosk
• Please help yourself to coffee and take a seat
• Bathrooms are right next to the reception area

Parking Instructions for Optimal Brain MN – Oakdale Clinic

Address: 992 Inwood Ave North, Oakdale, MN 55128

Step 1: Arrival

• The clinic is located just off Inwood Ave N
• Look for the Optimal Brain MN sign on the building. We are the corner building and the building is cream and brown. Some say, it looks like condos.

Step 2: Parking Lot Access

• Turn into the shared clinic/office complex lot directly in front of the building.
• Parking is free for patients.

Step 3: Choose a Parking Spot

• You may park in any open, non-reserved spot in front of the building.
• Accessible (handicap) parking is available near the main entrance.

Step 4: Enter the Clinic

• Use the main front entrance of the building.
• Once inside, follow signs for Optimal Brain MN.
• Please sign in at the white kiosk so your provider knows you have arrived
• If you need to complete intake paperwork, these can be found next to the white kiosk
• Please help yourself to coffee and take a seat
• Bathrooms are down the 1^st floor hallway and to the left

Address: 5775 Wayzata Blvd, Suite 700, Minneapolis, MN 55416

Step 1: Arrival

• The clinic is located in the Park Place West building, just off Wayzata Blvd near the Hwy 100 and I-394 interchange.
• Look for the 5775 Wayzata Blvd office building.

Step 2: Parking Options

• Visitor parking lot: Located directly in front of the building (surface lot).
• Parking ramp: Additional parking is available in the attached ramp next to the building. Visitor spots are marked.
• All patient parking is free.

Step 3: Enter the Building

• Use the main lobby entrance from the parking lot.
• Elevators are located straight ahead in the lobby.

Step 4: Go to Suite 700

• Take the elevator to the 7th floor.
• Follow the signs to Suite 700 – please check in with the receptionist, and take a seat
